This service addresses the gap between impatient and community care, providing a personal and tailored support base for service users prior to returning to independent living. 

Patients are admitted from hospital for a short - term only for extra social/nursing assistance to facilitate a more safe return home.

The aim is to reduce unnecessary admissions and give people the support they need to remain independent.

This step down unit transition to a lower level 2 care will provide an intermediate level of care for patients with requirements between health and social care and intern will improve the efficiency of patient flow.

We aim at supporting people who are being discharged from an acute hospital but need a short - period of assessment to understand what their future care and support requirements are. This is a short term place placement where patients would receive care but not long enough to become dependent.
